Registration Information
How to Register
Fall 2014 registration dates:
Residents: Monday, August 18
Non-Residents: Monday, August 25
Classes begin Monday, September 8
Registrations which are received before
the above dates will be held and processed
on the day after registration begins.

2. Drop-off

A drop-off slot is located to the right of
the receptionist counter. Please, no cash
payments. Registrations are processed
Monday–Friday. Drop-off registration
is also accepted at the Prairie Lakes
Community Center, but is not processed
until the following business day.

4. Fax
Fax the completed registration form to
847-391-5707 with your complete credit
card information. Please be aware: if you
are registering online or via fax, your online
or facsimile signature shall substitute for,
and have the same legal effect as, an
original form signature.
5. Online Registration
Begins at 12:01a on 8/18 for residents,
and 12:01a on 8/25 for non-residents.

Register early to avoid
program cancellations!
1. If the program you choose is filled, you
will be given your second choice.
2. If both choices are filled, or you did not
list a second choice, you will be put on a
waiting list. Payment will be refunded two
weeks after the start of the class, or applied
to any outstanding balance you might have.
3. If an opening occurs, we will contact you
by phone. Re-registration may be required.
4. If the program is cancelled, or changes,
we will contact you by phone. Payment will
be refunded or applied to any outstanding
balance you have.
5. All registrations must be completed on
the registration form, with the waiver
signed.
6. Registrations received before the
registration date will be held and processed
on the day after registration begins.
About Non–Resident Program Fees
Persons living outside the Park District
boundaries can pay up to 25% more than
the resident rate for most programs and
facilities. Non–residents can also pay a
yearly fee of $150, which will give them
“resident” status.
Additional Payment Information
1. The Park District reserves the right to
change a payment fee in order to reflect
the correct fee.
2. If program fees differ between your 1st
and 2nd choice, please pay the higher fee.
If you are enrolled in the program with the
lower fee, we will refund the difference.
3. A $25 service charge is assessed on all
checks returned due to insufficient funds.
Scholarship Information
Program scholarships are available for
individuals unable to afford program fees.
Scholarship requests should be submitted
at least three weeks prior to all program
registration. An approved scholarship is
required for camp registration at a reduced
fee. Approved scholarship applications do
not guarantee acceptance into programs;
all general registration procedures should
be followed. Scholarship applications may
be obtained at the Leisure Center or Prairie
Lakes Community Center during business
hours, or by calling 847-391-5711.

Payment Plan Information
A $25. processing fee will be added for
each payment plan set sup.
No payment plans will be accepted after the
required registration date.
1. Fill out registration form and bring it to
the Administrative & Leisure Center.
2. An installment billing agreement will be
filled out and reviewed at the Administrative
and Leisure Center, by a Park District staff
member.
3. Installment payments can be done
electronically to a checking account,
savings account, or credit card. A routing
number and account number will be needed
for proper processing. Routing numbers can
be found on your check or monthly bank
statement.
4. A copy of the installment billing
agreement will be given to you, once it has
been approved.
Creative Corners Preschool
Payment Plans
You must register by September 2, 2014
to take advantage of the payment plan.
• A down payment of 1/4 is due at time
of registration.
• The balance is due in 3 equal payments:
1/4 each on Nov. 15, Jan. 15 and Mar 15.

Refund Policy
• 95% for all refunds received and
DATE STAMPED by September 2, 2014
• 90% refund for all refunds received and
DATE STAMPED after September 2, 2014
• After the first week of a class, no full
refunds will be granted unless the class is
cancelled. Refund requests received after
the first week of a class will be honored if
the request is due to a medical condition. A
doctor’s note must accompany the request.
