Have you completed your
Annual Renewal?
August 14 is the deadline to avoid suspension
Most of you have already completed your
annual renewal requirements and we
thank you for your prompt attention to this
important matter. For those of you who
missed the June 3rd deadline, we urge to
complete your annual renewal as soon
as possible to avoid suspension of your
certificate of registration. To see what is
outstanding, please log in to the secure
Member Site and access your online
information. The deadline to avoid a suspension is 5pm
Wednesday August 14th. Members who
miss this deadline will have their registration
suspended effective 12:01 am Thursday
August 15/19.
Once you log in, click on Complete My
Member Renewal and you will see what you
still need to complete. There are direct links
to those items. If you are unable to log in,
please contact Physician Advisory Services
at the number below, and we will be happy
to help. The online form outlines your payment
options. You will receive notification once
your renewal has been processed, after
which you will be able to print your receipt
and membership card from our secure
website.
PAYMENT INFORMATION:
To pay your annual fee online, please ensure
your internet browser is up-to-date (at a
minimum: Chrome v30, Firefox v27, Safari v7,
Internet Explorer v11).
