DR. MOHAMED SALIH IZZELDIN

On October 29, 2018, the Discipline Committee
found that Dr. Izzeldin committed an act of profes-
sional misconduct in that: he engaged in sexual abuse
and he engaged in an act or omission relevant to the
practice of medicine that, having regard to all the cir-
cumstances, would reasonably be regarded by mem-
bers as disgraceful, dishonourable or unprofessional.
The Committee found that Dr. Izzeldin engaged in
sexual abuse and disgraceful, dishonorable or unpro-
fessional conduct of three patients and engaged in
disgraceful, dishonorable or unprofessional conduct
with an additional six individuals.
Dr. Izzeldin's misconduct included three health
professionals (a registered nurse, a laboratory tech-
nician, a student medical laboratory technician in
training) and five particularly vulnerable patients.
Dr. Izzeldin’s behaviour to all of the complainants
had a similar pattern. With all the patients, there was
universally close body hugging and in some cases,
touching of other body parts outside of the clinically
relevant sphere of medical investigation at the time. In
three of the cases, there was clear sexual abuse of his
patients. While his behaviour constituted sexual abuse
in three of the nine patients, there appeared to be a
recurrent theme to the interaction with all nine indi-
viduals. Dr. Izzeldin made uninvited physical contact
with all of these patients and it appeared that the dis-
cussion involved remarks with a sexual theme, making
his patients and coworkers very uncomfortable.
Dr. Izzeldin’s physical actions of touching/holding/
initiating close body contact with patients and his
sexual banter were persistent and highly distressing
to these individuals and constituted sexual abuse of
Patients A, K and O.
ORDER
The Committee ordered: revocation of Dr. Izzeldin’s
certificate of registration; a reprimand; reimburse-

ment to the fund for therapy and counselling for
three patients for a total of $48,180, and post secu-
rity for that amount; and payment to the College for
hearing costs of $10, 180.

At the conclusion of the hearing, Dr. Izzeldin waived
his right to an appeal and the Committee administered
the public reprimand.
DR. ALLAN WOJCIECH JACKIEWICZ
PRACTICE LOCATION: Niagara Falls
AREA OF PRACTICE: Obstetrics/Gynecology
HEARING INFORMATION: Plea of No Contest, State-
ment of Uncontested Facts; Joint Submission on Penalty
On December 5, 2018, the Discipline Committee
found that Dr. Jackiewicz committed an act of pro-
fessional misconduct in that he has failed to maintain
the standard of practice of the profession and that he
is incompetent.
The allegations related to Dr. Jackiewicz’s gyne-
cological surgical practice and gynecological office
practice. The Committee found the following factors
to be of most concern:
1. The glaring lack of judgment in his practice over a
lengthy period, no apparent insight into his limi-
tations or deficiencies and no effort to remedy any
of his observed limitations or deficiencies.
2. The failure to follow accepted clinical practice
guidelines.
3. The failure to refer patients when appropriate.
4. The failure to educate patients about all treat-
ment options, including those he was not able to
provide.
5. The blatant disregard for ongoing education in his
field.
6. The surgical incompetence that was documented
by multiple observers and which may have led to
increased complication rates and harm to patients.
