Don’t be Late!
On April 16, 2018, the online feature for completing
your annual renewal will be available. As always,
it is critical that you provide us with your most up-
to-date contact information, including your email
address, fax and phone numbers. Before April 16,
please login to the secure section of our website
(Member/Applicant Login) and make sure that your
information is up-to-date. Important Information
We will be sending out paper notifications to only
those physicians who have not completed their
renewal in the first four weeks or so following the
first email notice that the survey is available. You
can view your renewal notification online by clicking
on My Documents within our secure website. Once
you have completed your renewal, and your fee
payment has been fully processed, you will receive
an email that your fee receipt as well as your
College membership card are available from our
secure website for you to print. By early May, if you haven’t completed your renewal
online, you will receive your renewal notification by
mail, with instructions for completing your renewal.

You do not have to submit your payment online;
the option to pay by cheque is still available.
Just complete and return the invoice page of
your notification along with your payment. Allow
adequate time for delivery to make sure your
payment is received by the College before the
deadline.
If you are going to pay online, your internet browser
must be relatively up-to-date. A change in the credit
card industry has led to a tightening of security
requirements in how payments are conducted. An
older browser will not allow you to submit your
payment to the College. (Please see the next page
for more information).
The deadline for completing your annual renewal,
which includes submitting both the survey and
payment, is 5 PM EDT on Friday, June 1, 2018. To
avoid any late fees, do not miss this important date.
