n election will be held on Tuesday, October 11, 2016 for the purpose of electing Council representatives to the College of Physicians and Surgeons of Ontario for Districts 1, 2, 3 and 4.
Members interested in becoming candidates must submit their nomination and conflict of interest
forms to the Registrar no later than 4:00 p.m., Tuesday, August 23, 2016.
The nomination form must bear the signature and registration numbers of at least five members eligible to vote in
the electoral district for which the candidate is nominated.* The candidate nomination and conflict of interest forms
are included in this issue. Additional forms can be obtained by calling Tanya Terzis at (416) 967-2600 ext. 545 or
1-800-268-7096 ext. 545, or by downloading them from the College website at www.cpso.on.ca.
Online voting will be open from Tuesday, September 20, 2016 to 4:00 p.m., Tuesday, October 11, 2016. Any
votes/ballots received later than 4:00 p.m. on October 11, 2016, will be considered invalid. Further information
about the voting process can be found at www.cpso.on.ca. Candidates will be notified of the results on Wednesday,
October 12, 2016.
