edical Center of Lewisville’s Senior Health
Clinic is committed to high-quality primary
medical care designed and dedicated to men
and women 65 years of age and older.
The clinic is staffed with specially trained healthcare
professionals who can assess, coordinate, and monitor
all aspects of your treatment and progress.
In addition, a social worker is available to assist patients
with remaining as independent as possible in their
living situations. They work directly with patients, their
families and physicians to assess individual needs and
provide appropriate referrals. Assistance can be provided
with such needs as medications, homemaker/personal
care, transportation, support groups, financial aid, and
much more.
How is the Senior Health Clinic different from a
primary care office?
Our Senior Health Clinic is
staffed by a specially trained
geriatrician – a physician who
is an expert in caring for aging
adults. Rakhshi Hydari, M.D.
is board certified in family
and geriatric medicine and
understands the unique needs of
seniors. “When a patient comes
for a visit, we look at all factors that impact health –
social and emotional factors, as well as the physical ones.
Our expertise and focus on senior care sets us apart from
other clinics,” said Dr. Hydari.

What services are available?
Our highly trained staff can perform lab
draws, medication refills, wellness visits,
and assist with accessing home health
agencies and other services to help
maintain your activities of daily living.
Does the Senior Health Clinic offer
other senior services, such as classes
or recreational activities?
Although we currently do not offer
these types of services, we can make
recommendations based on our
extensive knowledge of the area.
How can I become a patient?
Patients can self-refer (call us directly
for an appointment) or be referred by
a physician. As long as the patient is
65 or older and has Medicare or
Managed Medicare, our friendly staff is
happy to help schedule an appointment.
What should I bring to my
first appointment?
A form of identification (driver’s
license), your insurance card,
a complete list of your current
medications, and a copy of your
medical records (if available) will
help your first visit go smoothly.
