Another common criticism of the
standard Herbst appliance is that
because of its length, it allows for
too much vertical movement and
over rotation of the mandible
during sleep. As the jaw falls
open, while the muscles are
relaxed in N3 or REM sleep, the
mandible can fall open causing
the tongue to be too posterior
and inferior and promoting an
obstruction. Instead of raising
the Hyoid bone anterior and
superior, the Hyoid bone is
depressed losing the efficacy of
the appliance. The Respire Pink
Micro has a slight restriction
in the vertical movement and
therefore, prevents the mandible
from over-opening. The result
is greater support of the lower
jaw and an increase in overall
effectiveness.
