Community Health

Do you know how a Community Health Worker (CHW) can help you?

Karissa Frasier is a certified Community Health Worker in the Hays Cottonwood Extension District office, and she is here to assist you to better health. The best part- services are at no cost to you! Here are a few ways she can serve you:

•             Client Support: CHWs assist clients with setting goals and identifying barriers related to physical, social, and environmental health. Community Health Workers provide clients with community resources that can assist them with unique challenges.

•             Chronic Care Management: Community Health Workers help clients develop strategies to manage their chronic conditions so that they can live healthier lives. This 1:1 coaching is tailored to address individual obstacles.

•             Healthcare Liaison: CHWs learn about their clients’ needs and barriers to care. CHWs serve as navigators between clients, community, and health care systems.

•             Health Education: Community Health Workers provide culturally relevant health education to individuals and organizations to reduce risk factors and encourage healthy behaviors.

•             Advocacy: Community Health Workers recognize gaps that prevent client health, and advocate for individual and community health needs.

For example, you may be someone who struggles with hypertension. With the help of a Community Health Worker, you can identify lifestyle factors that prevent you from managing hypertension and find practices that improve blood pressure. Then, a CHW can educate, help you set goals, and connect you to local resources that can assist in achieving better health. Finally, your CHW can provide follow up care to assist you and keep you accountable on your journey.
