Form Modernization Overview Official Form 106G Overview: Official Form 106G, Schedule G: Executory Contracts and Unexpired Leases, replaces Official Form 6G, Executory Contracts and Unexpired Leases, in cases of individual debtors. New Form Elements: The form is simplified. Instead of requiring the debtor to make multiple assertions about each potential executory contract or unexpired lease, the form simply requires the debtor to identify the name and address of the other party to the contract or lease and to state what the contract or lease deals with. Definitions and examples of executory contracts and unexpired leases are included in the separate instructions for the form. NEW - Modernized Form Caption NEW – Amended checkbox REVISED - Official Form 106G, Schedule G: Executory Contracts and Unexpired Leases, replaces Official Form 6G, Executory Contracts and Unexpired Leases, in cases of individual debtors. NEW – Debtors are instructed to include contracts and leases listed on Schedule A/B. REVISED – There is now a place to list the address of any person or company with whom the debtor has a contract or lease REVISED - The revised schedule simply calls for purpose of contract or lease.
