Overview:
Official Form 122C-1: Chapter 13 Statement of Your Current Monthly Income replaces Official Form 22C-1,
Statement of Your Current Monthly Income in cases of individual debtors.
New Form Elements:
Official Form 122C-1’s form title and date modified have both been updated to reflect the 2015 forms
modernization project. Additionally, the form now incorporates a workspace column for debtor 2 pertaining
to questions #5 and #6.
