Overview:
REVISED – Official Form 122A-1: Chapter 7 Statement of Your Current Monthly Income replaces Official Form
22A-1, Statement of Your Current Monthly Income in cases of individual debtors.
New Form Elements:
Official Form 122A-1’s form title and date modified have both been updated to reflect the 2015 forms
modernization project. Additionally, the form now incorporates a workspace column for debtor 2 pertaining
to questions #5 and #6.
