Form Modernization Overview
Official Form 121
Overview:
Official Form 121, Statement About Your Social Security Numbers, replaces Official Form 21, Statement of
Social Security Number(s) in cases of individual debtors.
New Form Elements:
To make Form 121 easier to understand and complete, the form is divided into three sections, and
directions on the form are simplified. The debtor’s Employer Tax-Identification number (EIN) is eliminated
from the form, and the debtor’s name is moved from the caption to the body of the form.
REVISED – The language
has been revised, now
referencing Debtor 1 and
Debtor 2 in the caption.
The style also matches
the 2015 form changes
for modernized captions.
NEW – Part 1 was
formerly incorporated in
the caption, the debtors
name now goes in the
body of the form.
NEW – Part 2 allows a
debtor to enter in Social
Security Numbers
(section 2) and Taxpayer
Identification Numbers
(section 3). Employer’s
Tax Identification
Numbers are no longer
requested.
