Form Modernization Overview
Official Form 106E/F
Overview:
Official Form 106E/F, Schedule E/F: Creditors Who Have Unsecured Claims, consolidates information about priority and nonpriority
unsecured claims into a single form. It replaces Official Form 6E, Creditors Holding Unsecured Priority Claims, and Official Form 6F,
Creditors Holding Unsecured Nonpriority Claims, in cases of individual debtors.
New Form Elements:
Part 1 adds several new form elements. These include a modernized form caption, an amended checkbox, and a series of checkboxes to
indicate who incurred the debt, whether the claim is subject to an offset, and priority type.
Changed Locations:
Part 1 adjusts the location of several form elements. These include revised form title, a line specifically asking for the last four digits of
account numbers, and checkboxes to indicate whether listed claims are contingent, unliquidated, or disputed.
