Dear Area Resident:
Over the next three months your hospitals, health departments, and other health providers in
Marion, Ralls, Pike, Lewis, Shelby, and Monroe counties will conduct a Community Health Needs
Assessment (CHNA) so they can plan together to improve the health of our region. In order to
gather this information a confidential survey has been developed (for research purposes only) by
VVV Research & Development, Inc., an Olathe, KS based marketing research firm.
We value your input and ask that you take time to complete the enclosed survey. Results of
this assessment will be presented at an upcoming CHNA Town Hall meeting on
Thursday, June 20th at the Quality Inn in Hannibal from 5:30-7:00 p.m. We hope you will
be able to join us.
Note: If you prefer to fill out a survey online, please enter the following address into your
browser: https://www.surveymonkey.com/s/HannibalCHNA
Thank you in advance for your help in completing this confidential survey. The survey return
deadline is Tuesday, May 7th, 2013.
Sincerely,
Vince V. Vandehaar MBA
