VANGUARDS FACE TOUGH CHALLENGES AS
THEY CO-CREATE NEW MODELS OF CARE

sites are working to create a
network of care around the
populations that they serve,
Samantha Jones, NHS England
Director of New Models of
Care, told Health+Care 2015
This network of care will be built
around shared information shared
data based on population health and
integrated health working together.
The programmes involve clinical
engagement, patient involvement,
local ownership and national
support.
She said the work was challenging.
“The vanguards are finding it hard
because they want to move faster
than the system will allow them to.”
One barrier to overcome was the
current NHS payment system that
does not support a network of care.
NHS England is supporting the
vanguards in developing new NHS
payment and contracting models
which are multiyear capitated and
with incentives to deliver the new
models of population based care.
They also need support with IT and
governance. All of the vanguards,
whether they are Multi Specialty
Community Providers or Integrated
Primary and Acute Care Systems,
must create a single care record
working with IT and interoperability
across the health and social care
system.
“If we can deliver the shared care
model, people need to share the
information
in
real-time.
Interoperability between health and
social care is already happening in
parts the country but we need to
make it the norm.”
To succeed the vanguards need to
change the culture amongst the
people who are delivering care
locally. “We are supporting them to
keep talking about how that is being
delivered through something called
social movement which is basically
empowering and listening to people.
This is something I feel really
passionate about - it's about
supporting a different type of cultural
change for example helping people
to understand the language different
professional groups talk.
The workforce also needs to change
with roles blended and shaped to
meet the needs of population health.

change the workforce, but actually
anything is possible believe
