You need to fill this in at the end of every shift that starts after 1500
and finishes before 2200.
If there were two workers looking after one client, BOTH workers
needs to complete the shift Log when their shift finishes.

You need to fill this in at the end of every sleep over shift, whether
you were awake or asleep.
