To complete on the first day. Employer to keep photocopy.

From whom I must take instruction
How I am to sensibly and carefully follow instructions given to me
Who to ask if anything is wrong
What my duties will involve during work experience
When I am required to be at work – stable hours
Who I must call if I am sick or cannot attend work
The rules on drinking, drugs and smoking
Who I must see if I have any problems
Where the toilet facilities are
Where the first aid kit is kept
Where the health and safety policy is
The contents of the work experience/young people’s risk assessment
What the potential hazards are – horses, transport, machinery, etc
that I must wear any personal protective equipment provided
Safe lifting techniques have been demonstrated to me
What the fire procedure is
The accident reporting policy
Mobile phones and social media policy – understanding and respecting this
The Safeguarding policy and who to contact if I have concerns
About prohibited substances and cross contamination e.g. hay fever tablets
or using a topical gel such as ibuprofen
About inside information
I confirm that I have had the opportunity to ask questions and I understand the above.
