Endocare system on BESS, BMK Adequate Positioning of Inlets Dilators (5x200mm, 7x170mm) Due to the lumbar curve and a patient’ posture in prone position, the inlet markings on the skin could be different from those of the real entry at the level of the deeper interlamina space. Double check of the inlets under the C-arm after inserting smaller and serial Dilators, is needed. Remember the position and angle of the dilators to the target. After dilating the skin incisions and intermuscular space by Muscle detacher, somewhat more thicker-diametered, an endoscope can be easily inserted with correct angle to the exact target level. Function : It helps positioning the inlets on the skin under C-arm and lets the angle that an endoscope should be proceeded known exactly. Caution : Gently insert it. Forceful inserting can drive it over the ligametum flavum. Inlet confirmation Incise the two inlets on the skin and fascia about 1cm. Small one (5mm) is inserted first and the other bigger one (7mm) is followed overlying the first one. Small one moves out to the other inlet. Under C-arm, check the exact level. C-arm Remember the insertion angle of the dilators that you should insert an endoscope and an instruments with the same angle.
