‘ Consider triptans for headaches ’

Rachel Fieldhouse A NEUROLOGIST is encouraging GPs to consider trialling triptans for headaches that affect day-to-day function even without “ show-stopper ” migraine symptoms .
Dr Bronwyn Jenkins , a former president of the Australian and New Zealand Headache Society , said migraine was a very treatable condition but diagnoses were often missed if patients did not present with severe pain .
“ We expect these show-stopper ,
severe migraine attacks to be what we would call a migraine ,” the Sydney neurologist said .
“ But with anything more than a mild tension-type headache that they can function with , we are probably looking at someone with migraine disorder .
“ Some of the disabling features are associated symptoms , such as sensitivity to light or noise , nausea or even just inability to concentrate , exercise or have good energy levels . “ If it is limiting their activities
in any way — even if they can get to work or maintain relationships — it is still worth treating .”
For patients with moderate to severe pain or inadequate relief from simple analgesics , Dr Jenkins said triptans were a first-line option .
“ The biggest group of patients I see are trying simple analgesics , but these are never stopping the migraine headache effectively within an hour or two ,” she added . Five triptans — sumatriptan , eletriptan , zolmitriptan ,
naratriptan and rizatriptan — are available in Australia and PBS listed , while OTC formulations are also available .
For patients who could not tolerate triptans or had vascular risk factors , Dr Jenkins said medications such as rimegepant could prevent migraines by blocking calcitonin gene-related peptides .
Dr Jenkins has received consulting fees for education and advisory boards from Allergan , Eli Lilly , Novartis and Teva .

The judge found Dr Daynes had been rude and abrupt to her at times but rejected allegations he had sworn at her , thrown paperwork at her , regularly raised his voice , attempted to have her fired and tried to isolate her from other staff .
“ The evidence does not rise to the level of constant belittling ,” the judge said .
