In 1997 , in the course of their work in a specialist homeopathic clinic treating ear , nose and throat conditions in children , Jon Gamble and his colleagues noticed that , although generally successful and despite their confidence in the accuracy of their constitutional prescribing , some middle ear pathologies remained unchanged . He noted the same phenomenon when reviewing past cases from his clinic : “[ sometimes ] the classic reportorial approach had not yielded an effective remedy ”. This led him to reflect on a possible need to adopt different approaches to pathology on one hand and functional disturbance on the other : a constitutional remedy may not work on a pathology , that is , where there has been a long-standing change to the physical structure of tissue ( the example Gamble gives is emphysema , where lung tissue has been damaged ; whereas asthma is an example of functional disorder , where there is no damage but lung function is impaired ).

Of course , this is not to deny the ultimate importance of constitutional prescribing . The purpose of this book is to give practitioners protocols that will bring about rapid improvement in a presenting case while they seek a constitutional remedy to effect cure . The book is divided into seven parts : illnesses of children and women , general illnesses and illnesses of men ; case examples in treatment of chronic disease ; practice forms ; and appendices . These last two are handouts for practitioners to give their patients .
