Summary : Morning loaded calorie intake in humans has been advocated as a dietary strategy to improve weight loss . This is also supported by animal studies suggesting time of eating can prevent weight gain . However , the underlying mechanisms through which timing of eating could promote weight loss in humans are unclear . In a randomized crossover trial ( NCT03305237 ), 30 subjects with obesity / overweight underwent two 4-week calorierestricted but isoenergetic weight loss diets , with morning loaded or evening loaded calories ( 45 %: 35 %: 20 % versus 20 %: 35 %: 45 % calories at breakfast , lunch , and dinner , respectively ). We demonstrate no differences in total daily energy expenditure or resting metabolic rate related to the timing of calorie distribution , and no difference in weight loss . Participants consuming the morning loaded diet reported significantly lower hunger . Thus , morning loaded intake ( big breakfast ) may assist with compliance to weight loss regime through a greater suppression of appetite .

Background : To align with current best practices , manual therapists have refined their treatment options to include exercise and pain education for people with chronic musculoskeletal pain . In this commentary , we suggest that manual therapists should also add telehealth to their toolbox . Thus , we aim to discuss the use of telehealth by manual therapists caring for patients with musculoskeletal disorders .
Main body : Telehealth can be delivered to the patient in different modes , such as real-time clinical contact or asynchronously . Platforms vary from websites and smartphone apps to virtual reality systems . Telehealth may be an effective approach , especially for improving pain and function in people with musculoskeletal pain , and it has the potential to reduce the individual and socioeconomic burden of musculoskeletal conditions . However , the certainty of evidence reported in systematic reviews is often low . Factors such as convenience , flexibility , undivided attention from the clinician , user-friendly platforms , goal setting , and use of evidence-based information are all enablers for telehealth use and improving patients ' knowledge , self-efficacy , and self-management . Barriers to widening the use of telehealth in musculoskeletal care include the reliability of technology , data privacy issues , difficult to build therapeutic alliance , one-size-fits-all approaches , digital health literacy , and payment models .
Conclusion : We suggest that practitioners of manual medicine make telehealth part of their clinical toolbox where it makes sense and where there is evidence that it is beneficial for people who seek their care .

Endometriosis is a common gynecological disease , and its underlying mechanisms remain elusive . Patients are at a higher risk of recurrence after surgery or drug withdrawal . In this study , to identify a potentially effective and safe therapy for endometriosis , we screened potential target genes of kaempferol on endometriosis using network pharmacology and further validation . Network pharmacology showed kaempferol may suppress migratory and invasive properties by modulating the phosphoinositide 3-kinase ( PI3K ) pathway and its downstream target matrix metalloproteinase ( MMP ) 9 . Furthermore , in vitro experiments showed that kaempferol repressed the migration and invasion of endometrial cells , and this effect may be involved in mediating the PI3K-related genes , phosphatase and tensin homolog ( PTEN ) and MMP9 . Network pharmacology and in vitro experiments showed that kaempferol , repressed the implantation of endometrial cells and formation of ectopic lesions by inhibiting migration and invasion and regulating PTEN and MMP9 , which may be associated with the PI3K pathway .

Athletes represent a major part of dietary supplement users . This scoping review aims to explore the prevalence of dietary supplement use among athletes worldwide , most commonly used supplements , sources of information on dietary supplements and their reasons for use of these supplements . PubMed , CINAHL , MEDLINE , and PsycInfo were searched for original research articles . Studies were included if they involved athletes , identified the prevalence of dietary supplement use , and were published after 2017 . A total of 26 articles were reviewed . Prevalence of dietary supplement use varied among articles , but sex-based differences related to the types of used dietary supplements existed . Generally , the findings were consistent in terms of reasons for use and sources of information . Unfortunately , the lack of homogeneity regarding the definition of dietary supplements , definition of use , reporting timeframes , and data collection methods complicates the attempt to compare the findings among studies .
