What do the grades mean? Premises that have received an outcome of Pass have met all of the expectations outlined in the OHP standards and will be scheduled for a reassessment within five years, earlier if additional procedures are added or a concern has been identified to the College. Premises who have received a Pass with Conditions outcome can remain operational; however, any identified deficiencies must be corrected to meet the OHP standards. Examples of such deficiencies may include inadequate patient records; additional patient monitoring may be required; and/or the maintenance of specified equipment. Premises that have received an outcome of Fail have posed a definite threat to patient health and safety and must cease performing the regulated procedures at the premises. Examples may include a major breach of infection control, non-qualified personnel; and failure to meet procedural standards. Failed premises are required to have a successful re-assessment before being allowed to perform the regulated procedure. Independent Health Facilities Program The Independent Health Facilities Program was established under the Independent Health Facilities Act. The Director of IHF (at the Ministry of Health and Long-Term Care) determines which facilities will be assessed each year. IHFs are assessed on a fiveyear cycle and given that no new IHF licenses have been issued in many years, the number of facility assessments is steady at approximately 200 a year. IHFs provide OHIP insured services, including services such as: 12 COLLEGE OF PHYSICIANS AND SURGEONS OF ONTARIO
