Facilitators (hand-outs that contain information on the topic of the client’s choice) are chosen by
the client and brought to each visit by the nurse home visitor. These facilitators help educate
clients about the importance of child immunizations, safety, and developmental needs of their
infants and toddlers.
Referrals are made to other community agencies based on the needs of the client.

Program Impact
Data from all NFP implementing agencies is collected through the national service office, and analyzed
to measure progress towards meeting implementation benchmarks for improving maternal and child
