of privileges during the past year or two ? If the only way an organization has to obtain this information is to ask the provider how many times , he / she exercised a specific privilege – the system needs to be updated .
8 . If the organization uses core / primary privileges , can the core / primary be modified by the applicant or by the organization ? Is the applicant asked to cross out or un-check anything in the core / primary that they don ’ t want to request ? How are the modified core / primary privileges communicated to those who monitor that providers exercise only those privileges that have been granted ?
9 . Does the medical staff office ( or other department handling privileging functions ) get constant phone calls to find out if a provider can exercise a specific privilege ? If yes , the privileging system isn ’ t well understood by those who must administer and monitor it .
10 . Finally , does the organization include blank lines on privilege forms so that providers can write-in / request privileges not included in the specialty or not yet delineated ? That should never be done . If a provider wants to request a privilege not currently included in a specialty or not performed at the organization , there should be a separate process to request the addition of the service and / or procedures to the privilege forms first – not a blank line on a privilege form for the provider to write-in a privilege .
