Case logs , procedure reports , ongoing or periodic professional practice evaluations and peer reviews all go into building a provider profile to ensure they are qualified to continue providing care under their granted privileges . Meeting the requirements established by the medical staff and accreditation bodies to confirm current competency is critical in ensuring patient safety .
