Credentialing and Privileging Checklist
There are a number of documents that must be obtained during the credentialing and privileging process and used to make credentialing and privileging decisions . A list of those includes , but may not be limited to ( based on the organization ’ s accreditation and / or what an organization deems as best practice per their policy ):
Primary source verification of the following :
• Education and training
• Board certification
• State licensure
• Work history and affiliations
• National Practitioner Data Bank ( NPDB )
• Office of Inspector General ( OIG ) Sanctions
• Malpractice and / or liability history
• Background checks and criminal history
• Registration and certifications
• Peer references
• Volume activity
• Peer Review / PPE outcomes
