• The final component of the privilege form is a section for a clinical recommender ( e . g ., Specialty Chief , Department Chair , Medical Director , Chief Medical Officer , etc .) to document and sign attesting to their review , evaluation and recommendation of the provider ’ s requested privileges . The recommendation would include any privilege conditions , modifications , or deletions as well as any explanation the clinical reviewer wants to make regarding the recommendation of the privileges for the provider .
