procedures that a provider must meet ; and a process for determining if the following sufficient resources are available within a specified time frame to support each privilege : 1 ) Space ; 2 ) Equipment ; 3 ) Staffing ; and 4 ) Finances .
At a minimum , in order to meet the Centers for Medicare and Medicaid Services ( CMS ) requirements , an organization ’ s delineation of Privileges should be criteria-based and contain requirements for education , training , board certification , and current competency for both initial privileging and renewal of privileges and any additional qualifications / criteria . Below is an example of what might be listed under the Required Qualifications section of an organization ’ s delineation of privileges .
• Education and training
• Medical school
• Residency / Fellowship
• Manufacturer Designated Training
• Board certification ( as required per medical staff bylaws )
• Maintenance of Certification ( MOC )
• Certificate of additional qualifications
