NEW DIAMOND GOLDEN SILVER SOROR FORM
Sorors initiated in 1945 (for Diamond), 1970 (for Golden) and 1995 (for Silver) are eligible to be
honored and receive a special souvenir at the Dorothy Buckhanan Wilson Conference Luncheon.
This form should be completed for each soror to be honored.
All honorees must register and submit the $15 fee in the online/electronic registration system.
Honorees are also asked to submit a 4 x 6 jpeg color photo for the luncheon video. The name of the honoree
should be included in the jpeg file name.
The deadline to submit this form, submit $15 fee and submit a color photo to be included in the program
presentation and to be recognized during the luncheon is February 8, 2020. No exceptions!
