TWO( 2) MEDICAL DOCUMENTS:

� 1) A completed CCS Family Athletic Authorization for Participation in Athletics form

� 2) Physician Clearance for Sports We will accept either option below as long as: 1) the form is signed on or after July 1 of the current school year, and 2) the most recent physical exam was completed within 12 months.

� OPTION 1: A completed State of CT Health Assessment Record( blue form) with clearance for all sports

� OPTION 2: An Alternate Sports Participation Form provided by a physician, as long as it indicates the student’ s name, date of birth, date of most recent physical, and clearance for all sports.

TWO( 2) CONCUSSION CERTIFICATES( must be completed annually)

� 1) Concussion in Sports Certificate of Completion( Parent / Guardian)

� 2) Concussion for Students Certificate of Completion( Student in Grade 6-12) PAYMENT
