10 Proof of Receipt of three hepatitis X b vaccines( two if Heplisav vaccine)
Students must upload documentation showing receipt of two / three vaccines.

11 Results of a hepatitis b titer( obtained after completion of initial 2 or 3 vaccine series)
Students must upload documentation of the results of a hepatitis b titer in addition to the proof of receipt of the vaccines
