Cardiopulmonary Resuscitation( CPR) Certification or re-certification is required before beginning clinical education experiences in any health care facility. Copies must be submitted to the Director of Clinical Education and placed in the student’ s file.
Certified Background Check: All students enrolled in the Radiation Therapy Program are required to have a criminal background check done through www. castlebranch. com. There are in-state( PA) and out-of-state checks done depending on where the student lives. Students will receive instructions on how to retrieve this information with their orientation letters which are emailed to them in the beginning of July. The student is responsible for the fee incurred.
A copy of the completed criminal background check is to be sent to the Radiation Therapy Program Director prior to the start of clinical.
* Some clinical sites will require additional criminal background checks such as Castle Branch Bridges or IDEMIA( FBI fingerprinting).
A certificate of health from a nurse practitioner or a family physician is necessary. All students are required to have a history and physical done prior to entering the program and clinical education. Students whose History and Physical Record are not on file will not be permitted to attend clinical education. The completed health form must be returned to the University nurse before August 1st. Students must provide documentation of all required inoculations. The Hepatitis B vaccination is required by the clinical affiliates. If the student chooses not to be inoculated for Hepatitis B, they must sign a declination form, which also must be returned to the University nurse. All history and physical forms and declination forms will be kept on file in the University nurse’ s office. The student must have the following or provide documentation of immunity to comply with Gwynedd Mercy University Radiation Therapy program. Students without completed health forms will not be permitted in any clinical site.
The Following Proof of Immunizations Are Mandatory: 1. MEASLES( RUBEOLA), MUMPS, AND RUBELLA( German Measles) 2. VARICELLA: Proof of two doses of vaccine OR serologic evidence of immunity( titer) 3. HEPATITIS B: Proof of three dose series of vaccine AND serologic evidence of immunity. Students with low / negative titer results after completion of three dose series should discuss reimmunization with a second three dose series, according to CDC recommendations for healthcare providers. A second serologic titer does not need to be submitted after the second three-dose series of vaccine.
All students must have begun the Hepatitis B Vaccine series prior to the start of classes. The series consists of three injections: an initial injection, followed by the second injection one month later, followed by the third injection five months later.
All students who have already completed the Hepatitis B vaccine series are required to have a Hepatitis B surface antibody titer( blood test) to confirm immunity. A copy of the lab report must be attached to this form. For students who are just beginning the Hepatitis B series, a titer is required one to two months after completion of the series.
