High volume center for TCAR
Vascular surgeons on the hospital ’ s medical staff are in a small national group of surgeons trained in transcarotid artery revascularization ( TCAR ). TCAR combines the safety and reliability of open carotid endarterectomy with the less invasive features of carotid stenting . Baylor Scott & White Heart and Vascular Hospital – Dallas is one of the highest procedure volume TCAR sites in the nation . As such , it has become a TCAR training site for vascular surgeons from around the country .

Patients who receive TCAR experienced positive outcomes as was noted in a study presented at the Society for Vascular Surgeons annual conference . Overall results appear to be better than traditional carotid endarterectomy and carotid surgery with the greatly reduced potential of strokes and myocardial infarctions . The Dallas site is one of the top volume sites for TCAR in the US with more than 550 total TCAR procedures completed since 2017 , the year TCAR began . Nationally recognized TCAR vascular surgeon on the medical staff , Bradley Grimsley , MD , is known for his national proctoring of others for making the TCAR procedure a treatment option available to more patients .

The first rib resection procedure involves removing part of the first rib , eliminating the pressure on the thoracic outlet . In some cases , blood vessels around the area need to be rerouted . If the vessels are extremely narrowed , angioplasty may become an option .

Pearl , MD , FACS , comedical director for vascular surgery at Baylor Scott & White Heart and Vascular Hospital – Dallas and Baylor University Medical Center through December 2022 . Dr . Pearl has focused on developing the multidisciplinary Thoracic Outlet Center , mentoring other vascular surgeons . For more on TOS Center , check out page 20 .
The TOS Center will continue to study outcomes , following patients for several years . Specifically studying and documenting the history of the program ’ s success with professional athletes , several notable presentations at national conferences occurred .
