1 Employees are required to make plan choices annually even when the enrollment choice is “ No Change ” 2 For life events other than birth and adoption , the effective date of coverage is the first of the month following receipt of your enrollment form and required
documentation as outlined on page 6 .

WHEN DOES MY COVERAGE B EGIN ?

For new hires , you must elect benefits within your first 30 days of employment , and your coverage will be effective your first date of hire . For employees that select coverage during the annual enrollment period , your coverage will become effective January 1 of the following year .
