Minimum of 30 returned surveys is required .

95 % minimum Overall Awards score for two consecutive years ( measured annually-January 1 , 2019 to January 3 , 2020 and January 1 , 2020 to January 5 , 2021 ). Minimum 50 % survey return rate ( measured between January 1 , 2020 and January 5 , 2021 ). Must have achieved a Quality Service award or a Quality Service Pinnacle award in 2019 for eligibility for the Quality Service Pinnacle award in 2020 .
