Senior Care Plus ( SCP ) is changing healthcare for seniors in Nevada . As the largest Medicare Advantage Plan in northern Nevada , SCP prioritizes members ’ health and happiness . We do that by bringing new members into a no-cost , comprehensive health assessment . During that initial appointment , a geriatric specialist focuses on the four Ms ( mind , medications , mobility and what matters to you ) to help build a personalized care plan for our members .
One of the many reasons we offer these health assessments , as well as an annual visit , free of charge is to help our team Build Relationships to Improve Care . This approach , also known as the BRIC Model , is the care model used across Renown . “ What ’ s special about this care model is that it really puts the member at the forefront ,” Savannah Gonsalves , a registered nurse with SCP , said . “ We have providers , nurses , Senior Care Plus personal assistances and case managers , and within the BRIC Model , they ’ re all talking to one another and putting the focus on meeting the member ’ s needs .”
