[Epub ahead of print] PMID: 31339519 OUTREACH TO THE COMMUNITY & THE UNDERSERVED Born from an academic practice partnership (Maria Shirey, PhD, RN, and Connie White-Williams, PhD, RN) between UAB Hospital and UAB School of Nursing, the Heart FailuRe Transitional Care Services for Adults (HRTSA) clinic was funded by a federal Health Resources & Services Administration (HRSA) grant from 2014 to 2017 and has been sustained by UAB Hospital ever since. The clinic is an interprofessional collaborative practice that brings many disciplines and areas together, such as medicine (with collaborating physician Vera Bittner, MD); nursing (nurse practitioners, clinical nurse leaders, registered nurses, and certified medical assistants); social work; behavioral health; pharmacy; the School of Health Professions; and the School of Public Health. The clinic is the medical home for underserved adult patients with heart failure who have no insurance, are under-insured, or lack a health care provider. It provides evidence- based guidelines-directed medical care and utilizes a transitional care coordination model, which includes rounding in the hospital, clinic visits, and a home visit. In addition, HRTSA uses a patient activation bundle that addresses social determinants of health. Behavioral health is integrated into patient care. Outcomes have been tracked since inception. Patients’ physical and mental health improves, patients become more engaged in their self-care, and they report 96% satisfaction with the clinic. Annual inpatient days among these underserved patients have been reduced from 1,294 pre-HRTSA to 266 in the most recent year. 30 UAB Cardiovascular Institute Annual Report uabmedicine.org 31
