Halifax Health - Center for Oncology provides all of our Navigators as a complimentary service .
Breast Cancer Navigator A diagnosis of breast cancer can be overwhelming for patients , as well as their families . Our Breast Navigator will provide emotional support , education , and act as a resource so that patients can make well-informed decisions regarding their care options . This service coordinates access to cancer care services while providing quality care . The Breast Navigator can assist with coping strategies for dealing with fear and anxiety that can be challenging for newly diagnosed cancer patients and their families .
Head / Neck & Lung Navigator The Nurse Navigator for Head / Neck and Lung Cancer patients at Halifax Health - Center for Oncology is an integral part of your treatment team . The Navigator helps guide patients through treatment , assist in organizing their care , and providing education and assistance each step of the way , making sure that patients get the support and help they need .
The Head / Neck and Lung Nurse Navigator empowers our patients by supplying them with current information and education regarding diagnosis , cancer treatment , management of any side effects of treatment and available resources . Patients receive the appropriate monitoring and self-care education to ensure the highest quality of life possible during their journey .
Hereditary Risk and Genetic Navigator Our Hereditary Risk and Genetic Navigator will assess individuals with a personal and / or family history of cancer to determine if genetic testing is right for them . Majority of cancers occur by chance , or are sporadic . However , some cancers are caused by an inherited gene mutation , or are hereditary . Identifying a hereditary predisposition to cancer allows individuals and families to personalize cancer screening guidelines , medical management , and treatment options .
The hereditary risk / genetic navigator empowers patients by supplying them with current information and education regarding your results . The navigators ’ goal is to help identify increased risk based on family history , the psychosocial impact of the history on patients and family , and the environmental factors and physical assessment data that contribute to disease or risk .
Oncology Rehabilitation Navigator The role of the Oncology Rehabilitation Navigator is to detect , mitigate and help treat functional decline of cancer patients . Cancer and its treatment can often impair normal daily activities . Whether it ’ s fatigue , neuropathy , pain , weakness , mobility difficulties , memory or attention difficulties , exacerbations of old injuries , or anything that can impair activities of daily living , our Oncology Rehabilitation Navigator is here to help .
Survivorship Navigation At Halifax Health , we know a patient ’ s journey with cancer doesn ’ t end when treatment is complete . Through our Survivorship Nurse Navigator program , we will continue this journey as a partner in our patients care . Our Survivorship Nurse Navigator is dedicated to helping after completion of their treatment and will serve as a system navigator , patient educator and advocate , care coordinator and community outreach connection . The Survivorship Nurse Navigator is the champion liaison of our patients recovery .
Our Survivorship Nurse Navigator provides a Care Plan listing all prior treatments and future appointments , which empowers patients with information on all aspects of posttherapy recovery . The Survivorship Nurse Navigator is available after treatment to provide advice and support as patients progress through recovery . They work closely with physicians to provide real-time answers to thier questions .
