Primary Care LEAD Team

YEAR ONE SUCCESSES :
A total of 40 pediatric providers , including pediatricians and pediatric ARNPs participated , representing 5 practices and 12 locations across Polk County .
A total of 2,236 prescriptions were disseminated and 3,999 goals were selected by patients .
The Primary Care LEAD Team has been addressing the prevention and reduction of childhood obesity using the 5-2-1-0 prescription program . This program provides simple steps children can take to achieve and maintain a healthy weight by developing healthier habits that will last a lifetime . The program launched in January of 2017 and has gained such momentum that the Team extended their plans through 2018 .
We would especially like to recognize the two practices who disseminated over 97 % of the prescriptions ( 2,179 prescriptions ):
• Central Florida Health Care ( 1,016 prescriptions )
• Lakeside Pediatrics ( 1,163 prescriptions )
The most popular goal selected was eating 5 servings of fruits and vegetables each day .

The 5-2-1-0 Prescription for Healthy
Living is an excellent tool to initiate conversations about healthy habits . This campaign is not limited by age and has been shared with many Polk Vision Teams and utilized by Polk County Head Start , Polk County Public Schools both in the curriculum and food service , Primary Care Providers , Florida Southern Nursing Program , and the Special Supplemental Nutrition Program for Women , Infants and Children , ( WIC ). 5-2-1-0 is a positive healthy message that can be conveyed throughout our county .”
