Setting/Delivery Mechanism Providers – Children’s Colorado as well as other community health clinic and pediatric practice providers will be educated regarding the importance of addressing healthy lifestyle behaviors, how to educate, empower, and motivate families to make behavior changes, and how to access and utilize topic specific resources from the resource hub as need be. Schools – Children’s Colorado school nurse team as well as other school based providers will be educated regarding the importance of addressing healthy lifestyle behaviors and how to access and utilize topic specific resources from the resource hub as need be. Employers – Partner with Corporate and Community Relations team to embed healthy lifestyle content into corporate lunch and learn program and other venues. Community – Children’s Health Advocacy Institute (CHAI) Prevention, Education, Outreach, and Coordination team will embed content as appropriate in current CHAI healthy lifestyle programming (ex – Camp Champions) as well as leverage existing programs and partnerships as a vehicle to reach parents. (ex – YMCAs, Boys & Girls Clubs, Corporate and Community Relations partners). Home – Hospital and community based community health workers and educators will be trained and educated regarding the importance of addressing healthy lifestyle behaviors and how to access and utilize topic specific resources from the resource hub as need be. Additionally, the community health team may use the content and toolkits for parent education sessions (1:1 or class setting).
