STATE-OF-THE-HEART CARE Heart disease is the leading cause of death in our state, claiming the lives of nearly 5,000 Nevadans every year. Renown Institute for Heart & Vascular Health is leading the fight to reduce this health burden and help our families, friends and neighbors live longer, healthier lives. As the region’s hub for heart and vascular care, we are on the forefront of treatments and support — from rapid, heart muscle-saving procedures to ongoing condition management to comprehensive wellness and prevention programs. In addition, our team of heart specialists is the largest in the region and serves eight local communities across northern Nevada — making sure that when it comes to heart care, we’ve got you covered. THE HEALTHIER PATH FROM HOSPITAL TO HOME Nearly 1 in 5 Medicare patients hospitalized with a heart attack is readmitted within 30 days of discharge. The same goes for 1 in 4 of those hospitalized with heart failure, adding up to $41.3 billion in annual hospital costs nationwide. By improving medication management and empowering patients to take a more active role in their recovery, a large percentage of these readmissions could be avoided. 50 That’s why our Patient Navigator Program was created. And because of the program’s success, we are one of 35 hospitals chosen to participate in a new national collaboration to improve heart care. This effort allows our team to work in partnership with such leading national programs as Vanderbilt Heart and Vascular Institute and Ronald Reagan UCLA Medical Center. “OUR TEAM APPROACH HELPS IMPROVE OUTCOMES FOR PATIENTS DEALING WITH SERIOUS HEART CONDITIONS.” — Christopher M.
