measurements have been taken, BiaCare requires that the person who will be measuring complete the following
before a custom order will be accepted:
1) WATCH VIDEO: The person measuring agrees to watch the instructional videos at BiaCare.com.
2) READ WRITTEN INSTRUCTIONS: The person measuring agrees to read all of the written
instructions for measuring. Written instructions can be found on page 59, online at BiaCare.com,
or emailed/faxed by contacting Sales@BiaCare.com.
Please signify that the above has been completed by checking the corresponding box on the order form. Orders
will not accepted without all boxes being checked. Your assistance in this will help the patient receive a better
